My Covid-19 Vaccination Status: 1st – 09/01/21. 2nd – 24/03/21.
Updated Covid Guidance from Jan 2022
Request Patients :
1. Inform me of ANY cold/flu types symptoms prior to appointment so that alternative dates/times may be offered if needed.
2. Conduct a LF test I hour before appointment and supply evidence of a negative result.
3. Wear a mask and use hand sanitiser at appointment.
These steps are in line with Government and Professional guidelines.

I am unable to offer appointments to:
- Children or young adults in education.
- Any person or member of their family that has been diagnosed with Covid-19.
- Any person or member of their family that has been contacted by NHS Test and Trace and told to isolate.
- Any person or member of their family travelling from abroad or a tier 3 location.
